- Evaluating and assessing referred patients.
- Developing an appropriate treatment plan, based on assessed needs and including objective, measurable short- and long-term goals.
- Applying and assessing the individual's response to treatment.
- Educating patients and caregivers about rehabilitation principles, interventions, equipment, and techniques.
- Participating in discharge planning.
The evaluation, assessment, and treatment are provided by FutureCare’s licensed Physical, Occupational, and Speech Therapists to achieve functional outcomes that:
- Promote health.
- Prevent or reduce disability.
- Maximize independent functioning.
- Facilitate the highest quality of life.
Our rehabilitation therapy services strive to restore each patient's highest possible level of functional independence through individualized treatment plans.
